REIMBURSEMENT OF REPAIR EXPENSES 1. Pursuant to an agreement between SONY BMG Music Entertainment and certain government offices, consumers can claim reimbursement for qualified out-of-pocket repair expenses incurred for their computers on which “XCP” content protection software was installed.2. Please print the attached form and complete it legibly, using blue or black ink. This form and any supporting documentation must be submitted by mail prior to December 31, 2007.
3. This program is intended to provide reimbursement for expenses incurred in repairing certain types of damage. SONY BMG reserves the right to reject any non-meritorious claim, including but not limited to any claim that is not based on a properly completed claim form, and any claim that is inconsistent with the release provisions of the class action settlement in In re SONY BMG CD Technologies Litigation, No. 05 CV 9575 (NRB) (S.D.N.Y.) (May 24, 2006).
4. SONY BMG reserves the right to investigate all claims and to advise state authorities of any claimant who provides false information in connection with a claim.
